TAEKWONDO CAMP - Waiver and Release

I do hereby agree to participate in Master Kang's TMA LLC located at 483 Hwy 79 Morganville, NJ 07751
I acknowledge the risks of injury that are common to any martial arts program that my child or I participate in, and I do hereby waive and release Master Kang's TMA from and against any and all claims, actions, damages, costs, liabilities, including attorney's fees and court costs, that may arise out of my participation in this program. I hereby execute this Waiver and Release form permitting my minor child and/or myself to participate in the Master Kang's TMA program.

I am aware that throughout my membership, there may be events and activities during which your child's participation, awards and/or honor may be videotaped, photographed or included in an article for a newspaper or on the school's website. I authorize the school to highlight my child's participation in this manner.

CAMP Policy

Refunds for absences are not provided.
Camper needs to bring your own personal drinks, snacks, and lunch.
We may have a short outdoor activity depending on the weather.
The camp fee must be fully paid before the camp starts.
Please wear the school t-shirt during your participation in the camp.
Student must wear fitness appropriate, which means no skirt, short shorts and tank top.
Student must arrive on time. Please notify us if any schedule occurs at the same time as our camp.
You will receive the field trip notifications for the respective week. (price, location, preparations)
Additional fees apply according to the field trip plans every Friday, separate from the camp fee.
No credits or refunds will be provided for missed camps.
Please drop off or pick up your child at the designated times.
