I agree to participate at Master Kang's TMA LLC located at 483 Hwy 79 Morganville, NJ 07751.

I acknowledge the inherent risks of injury associated with any martial arts program that my child or I participate in, and I waive and release Master Kang's TMA from any and all claims, actions, damages, costs, liabilities, including attorney's fees and court costs, that may arise from my participation in this program.

I hereby execute this Waiver and Release form to allow my minor child and/or myself to participate in the Master Kang's TMA program.

If I have any specific medical conditions, symptoms, or food allergies, I will inform the staff or master in advance and request appropriate measures to ensure safety. By providing information about any medical conditions or allergies for my child or myself, I aim to enhance safety.

During my membership period, I am aware that there may be events and activities where my child's participation, awards, and honors may be recorded on video, photographed, or featured in a newspaper or on the school's website. I authorize the school to highlight my child's participation in this manner.
